Here’s What We’ll Need From You

The following are the documents you must submit each year to continue being a contractor for ABC Home Services. We must have all of the documents returned to us by mail, email or fax by February 15th of each year.

• ABC Contractor Agreement  We need to have a new contract signed and returned to us each year.
• Certificate of Insurance. You can view a sample here of the Certificate of Insurance and the limits we need are available here.
• W-9. A blank form is available here.
• Current Workers Comp Certificate
